Diabetes Self-Management Education Program

Family HomeCare’s Diabetes Self-Management Program is recognized by the American Diabetes Association as meeting the national standards for excellence in diabetes education.

Overview
Diabetes mellitus is a chronic metabolic disorder. Individuals affected by diabetes must learn self-management skills and make lifestyle changes to effectively manage diabetes and avoid or delay the complications associated with this disorder.

For this reason, Self-Management Education is the cornerstone of treatment for all people with diabetes.

Family HomeCare’s program offers comprehensive instruction in the following content areas:

  • Diabetes Disease Process
  • Nutritional Management
  • Physical Activity
  • Medications
  • Monitoring and Using Results to Improve Control
  • Prevention Detection and Treatment of Acute Complications
  • Prevention Detection and Treatment of Chronic Complications
  • Goal Setting to Promote Health and Problem Solving for Daily Living
  • Psychosocial Adjustment to Daily Life
  • Preconception Care and Management During Pregnancy and Gestational Diabetic Management

Our Target Population:

  • Newly diagnosed with Diabetes (HgbA1C >8.5%)m
  • Home health patients of Family HomeCare
  • Type I or Type II Diabetics who are unstable or new to insulin

MISSION STATEMENT

Family HomeCare is dedicated to the delivery of a competent, effective and comprehensive self-management program to all individuals diagnosed with diabetes, without regard to race, sex, age, or socioeconomic background. We believe that those persons with diabetes require specific knowledge and skills to effectively manage their disease on a daily basis, and that self-management education is an integral component of the diabetes treatment plan. It is the aim of our Agency to provide a quality comprehensive program based on the ADA National Standards for Diabetes Self-Management Education Programs, by utilizing a multi-disciplinary approach, that is both cost-effective, and outcome measurable.

PROGRAM PHILOSOPHY

The philosophy of the program is to assist the individual with diabetes to maintain a high level of self-care by incorporating an individualized management regimen that will assist in the prevention of the long-term complications of diabetes, while still allowing for flexibility to enjoy a healthy, active lifestyle. In addition, Family HomeCare strongly supports the professional growth and development of health professionals involved in the care and education of our diabetic clients.
